Aim
The aim of the program is to help address the underlying root causes of a patient’s symptoms, including social, environmental, and economic factors by prescribing lifestyle changes and support patients to access a range of local, non-clinical services.

Outcome
Being a prescription (or referral) from a health care professional such as a GP or nurse, which doesn’t take the form of medicine or clinical treatment but prescribes walking and cycling and other lifestyle changes to patients, this program has a combination of outcomes. Such as securing funding to improve active modes infrastructure and campaign, hiring specific professionals (Social Prescribing Link Workers), increasing the number of people walking and cycling, increasing the feeling of well-being being described by citizens, and the possibility of long-lasting cross-sectoral collaboration of policy-making and funding availability.
